Search for LTC Homes ON a Map

The map below shows 14 different geographic areas of Ontario, covering the entire province. These areas are known as Local Health Integration Networks (LHINs).

Search instructions

  • To search for LTC homes in your area, select a LHIN location by clicking on the map.
  • You also have the option of scrolling down to the bottom of the page to select a LHIN from a table listing. When you hover over the name of the LHIN, the area of the map that it refers to will be highlighted.
  • Select the name of the LHIN and it will provide you with a listing of LTC Homes in that LHIN.
  • If you are unsure of what LHIN the location you are searching for resides in, you can do one of the following:

Local Health Integration Networks (LHINs)

LHINs are not-for-profit corporations that are responsible for planning, integrating and funding local health services in 14 different geographic areas of the province.

LHINs are based on a principle that community-based care is best planned, coordinated and funded in an integrated manner within the local community because local people are best able to determine their health service needs and priorities.

LHINs are intended to be the managers for health services that are delivered in hospitals, long-term care homes, community health centres, community support services and mental health agencies. For more information about (LHINs), see the Local Health Integration Networks (LHINs) section.
